Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.   • Job Summary: Provides case management and support to residents to ensure quality of care. Oversees the overall service plan to ensure resident maintains their highest level of functioning while receiving support that is appropriate. Works in collaboration with Interdisciplinary team at the direction of Director of Case Management.

    Responsible For Competency in the Following Areas:

    Initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs;

    Orient a new resident and family to the daily routine; assisting each resident to adjust to life in the facility;

    Assist each resident to maintain family and community ties and to develop new ones;

    Encourage resident participation in facility and community activities;

    Transfer/Discharge: Assists with discharges as requested by Case Management. Persistent efforts to follow up every 2 weeks with transfers to higher level of care. Document efforts within PCC

    Assist residents in need of alternative living arrangements to make and execute sound discharge or transfer plans

    Personal Care: Oversee the documentation and completion of assistance provided. Ensures shower schedules and service plan match and are per resident wishes. Follow up with resident refusals as appropriate. Ensure resident are clean, dressed appropriately and well manicured. Monitor and report significant changes in level of care to Director of Case Management.

    Assist the resident in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual's health and mental health needs;

    Assist the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident's health or mental health, including:

    (a) Health and mental health services;

    (b) Dental services; and

    (c) Medications;

    Resident Belongings: On admission and with any additions or deletions to resident belongings log. Update when residents and/or family make changes to the list.

    Walking Rounds: Remain visible and accessible to residents, staff and family members

    Resident Dining: Assists in a positive dining experience for the residents. Uses proper hand hygiene when handling food, passes meals in compliance. Observes and reports any residents who are not eating adequately, coughing with meals, or have noticed changes in their ability to eat or self-feed. Notify Director of Case Management

    Customer Service: Treats residents, families and co-workers with respect and dignity. Demonstrates good customer service, is warm and friendly.

    Appointments: Ensures residents are ready , properly dressed for the weather, ensures ALL residents have assistive devices as required. Residents going out with oxygen – ensure proper supply.

    Establish linkages with and arranging for services from public and private sources for income, health, mental health and social services;

    POC Documentation: Ensures documentation is complete and accurate

    Resident Choices and Preferences: Offers choices to residents regarding their care, times of care, choices in clothing, bathing ect. Respects and honors residents’ preferences. Promotes resident quality of life. Update service plan as appropriate

    Assist residents in making application for, and maintaining, income entitlements and public benefits;

    Assist in the establishment and operation of a system to enable residents to participate in planning for change or improvement in facility operations and programs and to present grievances and recommendations.

    Call Bells: Answers “All” residents’ call lights in a timely manner, anticipates residents’ needs and makes rounds on unit. Does not use “that is not my resident” approach and demonstrates the willingness to assists all residents and staff in need when requested.

    Accidents and Incident Reporting: Actively involved in reporting, investigation and follow up of all incidents and accidents

    A case manager shall have the following education and work experience:

    (i) a bachelor's or master's degree from an accredited college or university with major work in human resources or service delivery, and one year of full-time experience in the provision of services to a dependent adult population; or

    (ii) an associate degree from an accredited college or university with major work in human resources or service delivery, and three years of full-time experience in the provision of services to a dependent adult population.

    (5) A case manager employed in a facility prior to September 1, 1984 may be exempt from the requirements of paragraph (4) of this subdivision if he/she has a minimum of one year of experience in the provision of services to a dependent adult population.
