Case Manager, Sr. jobs in Tallahassee, FL

We are looking for an experienced Case Manager to join our team. The successful candidate will be responsible for providing case management services to clients, including assessing needs, developing individualized plans, and monitoring progress. The ideal candidate will have excellent communication and problem-solving skills, as well as a strong understanding of social services and the ability to work with a diverse range of individuals. Responsibilities:

• Develop and implement case plans that meet the needs of clients.
• Provide crisis intervention services to clients in need.
• Monitor client progress and evaluate outcomes.
• Maintain client records and prepare reports.
• Coordinate services with other agencies and organizations.
• Provide advocacy services for clients and their families.
• Develop and maintain relationships with referral sources.
• Educate clients on available resources.
