Case Worker - Home Care jobs in Atlanta, GA

Professional Case-Management Services of America (PCSA) currently has openings in DeKalb County and surrounding counties. Responsibilities for this position includes:

• Maintaining a caseload of 35 - 40 individuals
• Maintain a current knowledge of all state standards, guidelines, policy and procedures as related to service provision to the individuals with disabilities
• Communicate the mission and vision of PCSA to participants, families as well as community providers when conducting company business
• Attend meetings with families, providers or to the region when requested by the Area or General Manager
• To be detail-oriented, a requirement necessary for weekly documentation
• Be able to independently manage time to fulfill a productive workday while meeting established benchmarks.

Job Type: Full-time

Pay: $37,892.00 - $39,181.00 per year
