We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as a Case Manager. The ideal candidate will be responsible for overseeing and coordinating patient care, ensuring accuracy and efficiency throughout the care management process. The primary duties include recording case information, drafting service plans, coordinating care aspects, facilitating communication between stakeholders, and ensuring compliance with professional standards and regulatory requirements. The RN Case Manager will work closely with the healthcare team to review clinical records, verify documentation for billing, and maintain effective working relationships to promote quality patient care.

Responsibilities

• Assist with case management, including drafting service plans and reviewing case progress.
• Coordinate multiple care aspects, such as case coordination, information sharing, and discharge planning.
• Ensure timely completion and routing of visit requests to appropriate staff.
• Follow up on patient cases to ensure continuity and address emerging issues promptly.
• Oversee all patients to ensure care is initiated efficiently and in a timely manner.
• Review clinical records to ensure services align with patient care plans and regulatory requirements.
• Record case information accurately and complete all necessary forms.
• Verify and ensure timely signing of physician orders.
• Communicate effectively with primary care providers and insurance companies regarding patient conditions and treatment plans.
• Meet with direct care and administrative staff to communicate follow-ups or concerns regarding patients.
• Hold and document case conferences with other members of the healthcare team to ensure comprehensive care planning.
• Assist with verifying notes for billing accuracy.
• Ensure appropriate documentation resides in patient's clinical record for legal and regulatory compliance.
• Develop and maintain effective working relationships with clinical team members throughout the lifespan of case management.
• Ensure all deadlines are met in accordance with protocols, rules, and regulations.
• Adhere to professional standards as outlined by protocols, rules, and regulations.
• Abide by the agency's code of conduct to promote ethical behavior and maintain patient trust.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the state of Florida, as a Registered Nurse.
• One, or more, years of experience, in a Medicare Home Health Agency is preferred.
• Having knowledge of Medicare and Medicaid guidelines is preferred.
• Having working knowledge of home healthcare, and the principles and techniques of professional nursing, and required documentation that pertains to it is preferred.
• Should be skillful in organization, and in the principles of time management, and have knowledge of management processes.
• Must be able to contribute to the quality of care being rendered, through constructive communication with nursing managers and staff.
