Clinical Supervisor/Case Manager - Team Select Home Care

The RN Case Manager will split their time in the office and in the field with their assigned case load. They will prepare clinical documentation for each visit and summaries of care on patients following agency policy. This position will also make initial evaluation visits, reevaluate patients nursing needs, coordinate services, and more. The RN case manager will report to the Director of Clinical Services/Nursing Supervisor.

Clinical Supervisor/Case Manager Responsibilities:

• Perform comprehensive client assessments to ensure continuity of patient-focused care
• Create and implement clinical care plans and service assignments
• Ensure timely and accurate communication and documentation, both in the office and in the field
• Supervision of our direct client care team, nurses, and aides alike!
• Maintain confidentiality of all agency and patient-related information
• Ensure effective communication to patients, family members, and physicians
• Participate in staff meetings, required in-services, and agency activities
• Use effective time management skills to complete tasks in a timely manner

Clinical Supervisor/Case Manager Requirements:

• Current unencumbered Professional Nursing License
• Must have reliable transportation
• Proficient clinical skills
• Excellent verbal and written communication skills
• Ability to learn EMR software and basic functions of Microsoft Office
• Minimum of 1 year of nursing experience

Pay: $70,000.00 - $80,000.00 per year
