The RN Case Manager will split their time in the office and in the field with their assigned case load. They will prepare clinical documentation for each visit and summaries of care on patients following agency policy. This position will also make initial evaluation visits, reevaluate patients nursing needs, coordinate services, and more. The RN case manager will report to the Director of Clinical Services/Nursing Supervisor.
