Collaborates with insurers, managed care organizations, referral providers, patients, and families to assist developing case management guidelines. Leading Home Care has immediate openings for In-Home Caregivers / Direct Support Workers to work with clients in Lake Charles and the surrounding areas. Multiple shifts are available; we will match you to shifts that fit your availability. Applicants must be 18 years or older, must be able to read, write and follow instructions in English, and must satisfactorily pass all required and applicable State & Federal eligibility verifications, including background, driving, and other record checks.
Must possess a GPS-capable and web-enabled smartphone to log services per State & Federal EVV requirements.
Must maintain & provide copy of current Driver’s License or ID, Social Security Card, & Auto Insurance.
Specific physical requirements vary by assignment but may include bending, stooping, kneeling, reaching, standing, walking, pushing, pulling, carrying, and lifting up to 50lbs.
Must be punctual, dependable, respectful, patient, self-directing, able to work with little supervision, and willing and eager to fulfill accepted case assignments.
Must successfully complete orientation/training as required by State guidelines.
