Title: RN Case Manager

Schedule: FT, On Call, Holidays, Weekends

Location: Mesquite, NV

Position Description:

The Hospice RN delivers care within our patient home care programs. The RN will be responsible for providing care and services for Hospice patients. Visit patients wherever they reside which may include private residences, assisted living facilities, hospitals (pre-admission), long term care as well as group home environments.

Requirements:

Nevada State RN License in good standing

Reliable Transportation

Previous home health/hospice experience is required
