The RN Case Manager provides direct supervision of home health services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. Skilled nursing and other therapeutic services furnished are under the supervision of the Case Manager.
OUALIFICATIONS:
Graduate of an approved school of professional nursing and currently licensed in the state of Virginia.
Two (2) years nursing experience, preferred.
One (1) to two (2) years of home health (preferred) or recent experience in an acute care/rehabilitation setting providing care for the adult population.
Acceptance of philosophy and goals of Advance Care.
Possess excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
Proficient in basic computer skills.
Perform all nursing skills per competency checklist.
Ability to exercise initiative and utilize good independent judgment.
Demonstrates patience and maintains a professional demeanor at all times.
