We are seeking to find the perfect part-time Registered Nurse to join our Home Care Agency. A great opportunity for a nurse who is passionate about providing excellent care to clients. Along with independency of managing their own schedule.

Part-Time RN Case Manager Responsibilities Include:

Conduct initial and on-going assessments: This involves assessing the healthcare needs of clients when they first enter the home healthcare program and continuously monitoring their health and well-being throughout their care.

Telephone calls and visits: Maintain regular contact with clients and their families through phone calls and in-person visits. This ensures that clients are receiving appropriate care, and any concerns or issues are addressed promptly.

Develop comprehensive care plans: Based on the assessment of the client's needs, as the RN you are to develop a detailed care plan. The care plan will outline the specific care, treatments, and services that the client will receive, including medication management, wound care, therapy, and more.

Provide supervision and field training: This task involves overseeing and providing guidance to Certified Home Health Aides (CHHAs) who deliver care to clients. Training and supervision are crucial to ensure that CHHAs follow care plans and provide quality care.

Completion of necessary documentation: Accurate and thorough documentation is vital in healthcare. As the RN you are required to maintain detailed records of the care provided, the client's condition, and any changes or updates to the care plan. The accuracy of the documentation is essential for tracking progress and ensuring compliance with regulations.

Follow-up when needed: Follow-up on the care provided to ensure that clients are responding positively to treatment and that any necessary adjustments to the care plan are made promptly.

Traveling to clients' homes: In some cases, you are required to travel to the clients' homes to provide care and support. This can involve driving to various locations, which includes North Jersey.

Qualifications Include:

• Current New Jersey RN license
• Minimum two years Home Care experience
• Strong assessment, documentation, and organizational skills
• Ability to work independently
• Strong computer skills
• Must have own vehicle
• 2 Forms of Identification
• Proof of Automobile Insurance & Registration
• Results of Physical Exam (within the last 6 months)
• Results of 2-Step PPD (within the last 6 months) or Chest X-ray
• Proof of Mumps/Rubella/ Hep B/ Influenza

Job Type: Part-time

Pay: $40.00 - $50.00 per hour

Expected hours: 10 – 30 per week
