Minimum Qualifications:
 Required:

• Bachelor's Degree in Social Work; Nursing, Mental Health, Health & Human Services
• One year experience in HIV Case Management, Mental Health Social Services or Public Health programs; OR an equivalent combination of education and experience. 
• Must possess a valid Texas Driver’s license. 

Position Overview: 
Under basic supervision, provides case management services for HIV/STD patients of the Public Health District
(PHD) clinic; performs intake assessment and develops plan of care.

 Essential Functions:
 • Completes initial intake and assessment of clinic patients and develops plan of care.
 • Provides ongoing case management services.
 • Refers patients to other social service and health care agencies.
 • Maintains patient records and database
 • Evaluates client progress and lab tests and provides counseling services
