    This position acts as a clinically competent Masters of Social Work (MSW) who provides skilled evaluation, intervention and support for Home Health and Hospice patients in their home environment or care setting. Effectively interacts with patients, significant others, and other health team members, while maintaining professional standards, agency policy, state and federal rules/regulations and accrediting body standards. The MSW is flexible in assignment and hours so as to meet the needs of the departments. The MSW’s primary schedule is Monday through Friday and shares a weekend and holiday rotation
    Position Reports To:Hospice Supervisor and agency leadership
    Essential Functions:
    • Evaluates the patient's biopsychosocial status, home environment, barriers to a safe and effective care plan, need for community resources and need for home health/hospice services through skilled assessment.
    • Interprets evaluation and test results and develops a plan of care with patient specific MSW goals to help patients develop, regain or maintain their ability to remain in their home environment and community.
    • Administers skilled treatment interventions as prescribed by the physician that may include, but are not limited to, short term counseling, resource support, grief and bereavement support, crisis intervention and development of advance directives.
    • Reassesses the status of the patient’s functional ability and the effectiveness of the social work plan of care and makes appropriate revisions with the physician’s approval.
    • Coordinates the delivery of social work services with nursing, aide, therapy, equipment suppliers, community resources and transitions of care which results in effective utilization of resources and timely attainment of achievable rehabilitation and/or palliative care goals.
    • Participates in weekly interdisciplinary team meetings and agency meetings as assigned.
    • Records, and reports to physician, the patient’s reaction to treatment, any changes in the patient’s condition, suggested changes in the patient’s orders and implementation of discharge planning procedures.
    • Maintains documentation of evaluation, reassessment, subsequent visits, coordination of care, discharge or transfer summaries and other clinical record documentation in the electronic medical record.
    • Instructs and supervises other health team members in social work procedures and techniques performed as an adjunct to the regular social work plan of care.
    • Required to maintain current BLS certification.
    • Other duties as assigned.

    Position Requirements:

    Qualifications:
    Required:
    • Masters in Social Work from an accredited program.
    • One year of work experience in a health care setting.
    • Must have LSWAIC (Licensed Social Work Associate & Independent Clinical) licensure.
      • ***With commitment to achieving LICSW (Licensed Independent Clinical Social Worker) or LASW (Licensed Advanced Social Worker) within the time-frame specified by the State of Washington.
    • Current valid driver's license and access to an insured vehicle with the minimum coverage by law.

    Desired:
    • Acute hospital experience.
    • Knowledge of community resources and referrals.
    • Understanding of medical terminology
    • Communication: Functionally able to listen, read, speak and write using English language. Functional hearing and visual acuity sufficient to detect changes in patient conditions and perform assessments. Effective interpersonal skills. Demonstrates ability to work in collaboration with health team members.
    • Must possess basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.
    • Licensure as LICSW or LASW
