Your Role
The Blue Shield Promise Social Services team is looking for a leader with Medi-Cal experience who can help facilitate the integration of social and behavioral needs care into overall healthcare to support members holistically. The Supervisor, Social Services Management, will report to the Manager, Social Services Management. In this role you will provide leadership, direction and are responsible for daily operations of the social services representative team. This team supports members with social service needs, navigates the health care system for linkages with healthcare providers, community resources, and administering social service assessments. The team also supports with triaging incoming social service Medi-Cal referrals and works in collaboration with the social services team. The supervisor oversees social services representatives serving members in both Los Angeles and San Diego County. The social services supervisor ensures all elements of the operations are in compliance with health plan and regulatory requirements as applicable to membership product lines and program benefits with special attention to the health and welfare of the members served. This position requires experience that focus on social service triaging, assessment, resource coordination, and managing Medi-Cal referrals.
