Home Health Occupational Therapist needed for Sun Prairie/Beaver Dam , Ideal schedule M-F 8-5. Home Health experience not required, training and advancement opportunities offered!
As a home health clinician with PRN HH&T you will visit patients in ALFs and in their homes. You will have the opportunity to observe the patients in their home environment. Many Home Health patients don’t see a lot of people, so you’ll also be an important part of their day!

Job Duties Include:
· Evaluating and developing individualized patient treatment plans
· Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
· Instruct Certified Occupational Therapist Assistants in treatment procedures to be utilized and provide supervision.
· Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
