We are a Skilled Nursing Psychiatric team looking for an energetic Occupational Therapist to join our team! We provide skilled and compassionate home care for patient’s behavioral and medical health needs through innovative and client centered approaches.

REQUIREMENTS

• Evaluates and establishes goals based on the patient’s current level of functioning and potential for improvement and performs re-evaluations as indicated.
• Guides and instructs the patient in prescribed therapeutic, self-care, and creative activities that are directed toward improving independence and physical and mental functioning.

• Communicates with patient and family concerning medical history ,progression of treatment, and use of prosthetic, orthotic, and any other assistive devices.
• Communicates to physician the patient’s progress and, if needed, changes needed to the therapy program through periodic written summaries

• Documents per agency guidelines and in the required 24 hour time limit
• Participates in the Performance Improvement Committee process as requested; required monthly staff meeting; and assisting in the planning, operation and evaluation of the Occupational Therapy services of the organization.
