Cert. Occupational Therapist Asst. - Nursing Home jobs in New London, CT

Per diem position opportunity for a visiting Occupational Therapist to join an established and growing home health care agency.

Responsibilities:

• Perform home visits for patients in a home care setting.
• Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our client's plan of care.
• Observes, records, and coordinates care with multi-disciplines regarding the client's treatment and any change in the client's condition.
• Teaches, supervises, and counsels the client and family regarding home therapy procedures and other care needs as appropriate to the client's condition.
• Completes comprehensive OASIS-E assessments, transfers, resumption of care, and discharges following agency-defined time frames.

Qualifications:

• Current license as an Occupational Therapist in the state of Connecticut.
• Valid Drivers License
• Basic Life Support (BLS) 
• Reliable transportation and willing to travel in New London County and other surrounding towns
• Strong communication and interpersonal skills.
• Home care experience, current OASIS / Medicare experience and electronic health records experience preferred.
