The Registered Nurse Case Manager is a professional member of the home health care team, and is responsible for coordinating the plan of the interdisciplinary team, to best meet the needs of the patient, under the direction of the physician. Case Managers are trained to complete Comprehensive Assessments using OASIS, evaluate patient need for skilled nursing, and subsequently develop goals and deliver nursing intervention(s), including but not limited assessment and monitoring, teaching and training, wound care, infusion, catheter management, venipuncture, and or palliative care, to patients under the established plan of care. Case Managers are expected to adhere to all Fort Hudson Certified Home Health Agency Policies & Procedures.
