Pay and Benefits:
Pay rate is from $17-$19.50/hour (we offer raises every six months)
First Choice pays travel time mileage 
Flexible hours

We can guarantee hours up to 30/week

Responsibilities: 
Assists patients with their daily living activities
Spends time assisting patients with grooming to help them feel great
Works as part of the care team to help patients reach their goals and to be comforted

Always maintains patient comfort & safety
Attends case conference weekly to coordinate patient care with case manager and other clinicians

Qualifications:
Active CNA license

Valid Driver's license
Reliable transportation
