    Responsibilities

    Clinical Excellence/Patient Care

    ·Ensures quality and safe delivery of services to patients according to care plan.Uses equipment and supplies safely, effectively, and efficiently.

    ·Consistently utilizes a patient-centered approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.

    ·Treats patients/clients to relieve pain, develop or restore function and maintain maximum performance using specified occupational therapy procedures.

    ·Directs and aids patients/clients in plan of treatment.

    ·Utilizes physical agents including heat, cold, water, electricity, ultrasound and massage.

    ·Observes and reports to the Occupational Therapist and organizational personnel the patient/client reaction to treatment and any changes in patient/client condition.

    ·Instructs patient/family/caregivers in care and use of prosthetic device, therapeutic aids, splinting, and wrapping techniques as needed.

    ·Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.

    ·Assists in the promotion of services and products.

    ·Maintains accurate and timely documentation of clinical records and time/travel entries.

    Quality

    ·Delivers quality care consistent with Home Health Conditions of Participation, qualifying home care criteria, payor coverage criteria, and providing visits/treatments consistent with the plan of care.

    ·Conducts clinical review of records to include multi-disciplinary review, coordination of care, provided in conjunction with visits and documentation.

    ·Identifies risk for acute hospitalization and proactively prevents adverse events.

      • Participates in case conferences, in-service programs and other meetings required to ensure coordinated and comprehensive care. Achieve certification in Integrated Care Management within 1 year of hire. Certification includes completing the Integrated Care Management formal training session, successfully completing assigned computer based-learning modules within 2 weeks of training, and a competency evaluation. Use the Integrated Care Management philosophy, skills, tools and documentation in clinical practice. Demonstrate competency in selected ICM skills annually.
      • Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives.

    Basic UPH Performance Criteria

    ·Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.

    ·Demonstrates ability to meet business needs of department with regular, reliable attendance.

    ·Employee maintains current licenses and/or certifications required for the position.

    ·Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

    ·Completes all annual education and competency requirements within the calendar year.

    ·Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.Takes appropriate action on concerns reported by department staff related to compliance.


    Qualifications

    QUALIFICATIONS:

    Minimum Requirements Identify items that are minimally required to perform the essential functions of this position. Preferred or Specialized Not required to perform the essential functions of the position.

    Education:

    Licensed or otherwise regulated, if applicable, as an Occupational Therapist by the state in which practicing, unless licensure does not apply in the practicing state. Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE Eligible to take, or has successfully completed the entry-level certification examination for Occupational Therapists developed Master’s degree

    and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)

    Experience:

    One year experience caring for similar patient population as to be assigned. Home care experience 2 years’ experience

    License(s)/Certification(s):

    Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements. Mandatory Reporter: Child & Dependent Adult Abuse. Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities. CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification. Certification in programming applicable to home care population

    Knowledge/Skills/Abilities:

    Basic knowledge of current, frequently used medications, including modes of administration, actions and side effects. Able to perform occupational therapy assessment and treatment in the home setting without direct supervision. Strong interpersonal skills. Ability to work as a collaborative team member. Ability to understand and apply guidelines, policies and procedures. Ability to navigate and perform basic use of Microsoft Office products. Maintains a reliable mode of transportation and has the ability to drive safely during all day and night hours and in all types of inclement weather. Experience with documentation in an electronic health record.
