Minimum Educational

Associate degree in designated field

Certification Requirement

Basic Life Support (BLS) within 30 days of hire

License Requirement

Certified Occupational Therapy Assistant (COTA) license in the state of Kentucky

Optician, Dispensing license in the state of Kentucky
