The Case Management Specialist assists with discharge planning arrangements as directed by the RN Case Manager and or Director, which includes but not limited to faxing and copying the medical record to assist in transferring to the next level of care, and/or obtaining authorizations for lower level of care such as home health, SNF, or assisting in scheduling outpatient doctor appointments. Assists patients/families with completion of forms in regards to patient choice, etc. Data entry and logging of pertinent information, maintain department files, restocks forms and supplies, and other duties assigned. As directed composes and transcribes correspondence for the department Director.
Demonstrates a working understanding of federal and state regulations, departmental policies, particularly those related to discharge planning. High School Diploma or equivalent Minimum of 3-5 years of clerical experience required, preferable in a hospital setting.
