    About the Position:
    South Lane Mental Health is seeking an Alas Case Management Specialist to join our Alas Latinx Outreach Program. This position would be reporting to the Alas Program Manager, while also coordinating closely with our Outpatient Case Management Team. The Alas Case Management Specialist provides as-needed support to clients who require assistance with tasks such as attending appointments and meetings in the community, providing skills training, and other case management support.

    Position Duties and Responsibilities:

    Case Management responsibilities
    • Conduct individual and group skills training, case management, and/or peer support sessions with clients receiving Care Coordination services from SLMH, with a focus on increasing clients ability for self-management and shared decision-making, health literacy, and ability to access community resources with a goal of enhancing health and well-being and increasing the clients’ quality of life.
    • Provide education to the client about navigating complex systems to gain access to care and resources.
    • Provide education to others, including community service providers, regarding the case management process.
    • Serve as the contact point, advocate, and informational resource for client, care team, family/caregivers, and community resource agencies.
    • Assist with resolving issues that could affect treatment.
    • Participate in Complex Case Consultation meetings with Care Coordination Manager, Therapists/Case Managers, PMHNPs, and other pertinent staff to assist in developing interdisciplinary treatment plans, emergency plans, and other case management strategies.
    • Monitor client progress in a timely manner, and facilitate and communicate changes to plans as needed.
    • Assist in evaluating and monitoring the effectiveness of and adherence to the treatment plans.
    • Understand that many clients have been traumatized and strive to constantly provide services from a respectful, trauma-informed perspective.
    Administrative and continuing education responsibilities:
    • Complete all required paperwork, including notes on each session and individual client paperwork, in a timely manner.
    • Meet requirements regarding productivity and dependability.
    • Coordinate with co-workers and treatment teams, including participating in complex case reviews or family systems reviews as necessary.
    • Actively participate in clinical supervision, as well as staff meetings.
    • Engage in continuing education activities and trainings, and continually build therapeutic skills.
    • Participate in completeness and quality reviews as required.
    • As required, positively represent SLMH in the community and with other service providers, and/or act as a liaison with other organizations or with internal committees/cross-departmental teams.
    • Perform all duties in a respectful and responsible manner, both with clients and coworkers, and in a manner that meets all professional ethical standards.
    • Additional duties as requested.
    Position Requirements:
    • Bachelor’s degree in a social service-related field, or three years of relevant work, education, training, or experience in mental health required.
    • Designation as a Qualified Mental Health Associate in Lane County and ability to obtain MHACBO Registration required.
    • Verification of past employment and education, background check
    • Previous experience in case management, social services, or peer support preferred
    • Bilingual in English and Spanish or Mam preferred; bicultural preferred.

    • Ability to balance clinical, cultural, and ethical values, particularly as relates to client and staff needs.
    • Strong written and oral communication skills.
    • Ability to interact positively with people of all ages and cultural backgrounds.
    • Ability to work both independently and as part of a collaborative team environment.
    • Sound computer skills.
