Now Hiring - Chaplain- Lee's Summit, MO Area

Hours: M-F 8AM- 5PM

The Chaplain will be responsible for, but not limited to:

1. Provides pastoral or spiritual counseling in keeping with patient/family beliefs, and in accordance with Ascend Hospice policies and procedures.
2. Completes spiritual/pastoral assessment as indicated and appropriate, completed within 1 day of admission.
3. Develops an individualized spiritual/pastoral plan of care, according to regulations.
4. Offers patients and families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feeling, beliefs and values.
5. Responds to hospice patient/family needs as requested.
6. Coordinates and integrates pastoral services with other patient/family care services in order to ensure a continuum of care, including continuity of care following death or discharge.
7. Prays with families and patients, if and when desired by the patient and/or family unit.
8. Effectively participates as a member of the multidisciplinary Hospice team, providing quality Hospice care to the patient and his/her family.
9. Provides bereavement services, in accordance with Ascend Hospice policies and procedures. Initiates bereavement assessment between 7 and 14 days after the death of the patient.
10. Documents a plan of care for the bereaved survivors, based on the IDT's assessment.

Education:

Master's Degree in Divinity

Professional Experience:

• 2-4 years of Chaplain experience with end of life care
• At least 1 Unit of CPE completed
• Ability to provide non-denominational spiritual care
• Excellent written and verbal communication skills
