POSITION SUMMARY:
A vital member of the MI Choice team, this role is responsible for providing person-centered administrative support that ensures operational success in enrolling new participants in MI Choice Waiver. Provides scheduling of initial assessments, collection of participant and caregiver information, and data-entry and procedural processes required for enrollment. Works closely with the MI Choice Manager and care team members aiding in enrollment operations by completing assigned tasks and projects efficiently and with excellent customer service.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Provides telephonic collection of applicant and caregiver information.
· Conducts entry of assessment and person-centered service plan smart phrases for all initial assessments.
· Transfers relevant applicant information such as diagnoses and medications from health system electronic medical record to program participant record.
· Data enters information to assist in enrollment process including, but not limited to, nursing facility level of care and requests for authorization of services as recommended by Care Managers.
· Helps identify and resolve barriers to timely, efficient, and effective enrollment and start of services for applicants.
· Participates in training and professional development to ensure excellence in customer service and program delivery.
· Supports and maintains service line operations, compliance, records and reports by following policies and procedures.
· Documents in appropriate software system(s) and maintains current, accurate, and HIPAA compliant files and program records including document scanning and maintenance of electronic participant files.
· Actively participates in continuous quality improvement initiatives.
· Assists as needed with other assignments and projects that optimize implementation and development of service line operations and delivery.
· Other duties as assigned.
SUPERVISORY RESPONSIBILITIES:
No supervisory responsibilities of employees.
