Patient Enrollment Specialist conducts outbound calls to qualifying Medicare patients, with the intent to educate and enroll clinical patients in the Medicare Initiated Program of Chronic Care Management.

Job Description

This team member will perform a high call volume, predominantly outbound calling to Medicare patients. You will be the first point of contact to assist with patient education as you encourage the patient to enroll in their doctor's recommended Chronic Care Management (CCM) program.

It will be your goal to enroll patients and provide a smooth transfer to a Care Team Coordinator using our telehealth platform. We are searching for professionals with a sales or communications background.

Clear, confident speaking skills, ability to be trained in charting and a desire to connect with patients over the phone are essential to the Patient Enrollment Specialist role.

Essential duties:

• Make outbound calls to patients to educate and enroll them in the Chronic Care Management program (CCM) on behalf of their doctor.
• Based on a thorough understanding of patients' individual needs, provide education on ways CCM can benefit patients.
• Discover and act on opportunities to provide value in patient care.
• Some key performance indicators that your success will be based on include: number of daily contacts, patients enrolled/served, average call time, quality measurements and process adherence, ability to increase patient enrollment and education to the clinic you serve.

Minimum Qualifications

2 or more years in any of the following roles:

• High performing enrollment advisor significantly exceeding targets
• Patient care coordinator
• Any highly consultative, sales role

Preferred Qualifications

• Strong ability to demonstrate empathy, build relationships, discover patients' needs, and present CCM program effectively to enroll.
• Excellent communication skills (verbal & written)
• Growth minded professional
• Fluency in another language
• College education

Regular office hours are 8:30 am – 5:00 pm

Monday through Friday

*$15.00-$16.00 per hour
