The Parent Child Development Center in Waipahu is seeking to hire a full-time Care Coordinator to provide and coordinate services for infants and toddlers with developmental delay(s) in Wahiawa and North Shore communities.
This is a case management role with opportunities for utilizing advanced clinical skills in accordance with appropriate credentialling. Job responsibilities include, but are not limited to:
Meeting with families, providing information on the program, conducting assessments (intake, routine based, Battelle Developmental Inventory or Developmental Assessment of Young Children), coordinating home visits and meetings, developing Individual Family Support Plans and goals for the child's development, timely documentation, and connecting families to needed resources.
This position REQUIRES: A valid drivers license, reliable transportation and at minimum a Bachelors Degree in Social Work, Human Services, Psychology or related field.
MSW/LSW are highly desired. Salary offers commensurate with education, licensure and experience.
