    Your Specific Duties and Responsibilities:

    This position participates in health education activities related to sexually transmitted diseases and infections, treatment, and HIV prevention to include PrEP and nPEP in Area 4 which includes Duval, Baker, Clay, Nassau, and St Johns counties. This position functions as the HIV/AIDS Prevention Training and PrEP coordinator to clinic patients, staff, volunteers, and the community as part of Florida’s plan to eliminate HIV transmission and reduce HIV-related deaths. Conduct evidenced-based health education sessions to HIV positive persons and their partners on topics related to PrEP, nPEP, Test and Treat navigation and field testing for Syphilis and HIV, or other bacterial, biological, or viral agents.

    • Provides Medication Adherence Counseling.
    • Assists the client with physician instructions and compliance.
    • Performs patient care services such as monitoring patients for medication side effects, contra-indications, and non-compliance to prevent disease progression.
    • Provides family and patient education regarding the diagnosis of HIV/AIDS.
    • Makes referrals to case management, financial, and mental health counseling.
    • Establish working relationships with both internal and external customers including STD staff, HIV/AIDS staff, community-based organizations, and AIDS service organizations.
    • Provide specialized PrEP education to CBOs and community providers as needed or requested.
    • Conduct care planning activities (e.g., scheduling appointments, monitoring appointment adherence, preparing bi-annual care plans, identifying service gaps, case closure, etc.) for PrEP patients in partnership with their larger care team (e.g., medical provider, nursing staff, etc.).
    • Conduct PrEP education sessions with individuals at risk for HIV infection, including individual sessions, workshops, and social marketing activities for ongoing recruitment.

    Required Knowledge, Skills, and Abilities:

    • Knowledge of surveillance systems, research methodology, and data analysis.
    • Knowledge and understanding of HIV/AIDS including medical and psychosocial issues.
    • Knowledge of confidentiality of Protected Health Information (PHI) and adheres to guidelines.
    • Knowledge of HIV transmission, prevention strategies, and disease process.
    • Knowledge of PrEP and of high -impact prevention strategies to reduce HIV transmissions. Ability to communicate effectively and clearly both orally and in writing with a diverse population varying in age, physical/mental health, sexual orientation, socio-economic status, and cultural/ethnic backgrounds.
    • Strong organizational, time management, and project management skills with strong initiative and an enthusiastic attention to detail and accuracy.
    • Ability to establish and maintain effective working relationships with others, both internally and externally, and always represent the agency in a highly professional manner.
    • Ability to learn new information related to HIV/AIDS, related diseases, PrEP/nPEP, systems of care, and implement practices.
    • Ability to exercise independent judgment, solve problems, and make sound decisions.
    • Ability to work as a team member and accepts direction from a supervisor.

    Qualifications:

    Minimum:

    • Complete 500/501 HIV Counseling and Testing class within 3 months of hire.
    • A current and valid driver’s license.
    • Proficient in Microsoft Office 365 (Outlook, Excel, TEAMS, PowerPoint, etc).
    • Strong oral and written language and communication skills.
    • High School Diploma

    Preferred:

    • 500/501 HIV Counseling and Testing Certified.
    • 2-4 years of experience working in HIV/STI prevention or other community health programs.
    • Outreach engagement and community education experience.
    • Bachelor’s Degree or equivalent experience in social services.
    • Experience in public speaking.
