Family Educator/Case Manager

  • QUALIFICATIONS: Mental health paraprofessional with a bachelor's degree. 1 year of social service experience and/or group facilitation experience preferred. Computer skills, dependable transportation, and late afternoon/early evening hours required. Must be able to pass local and national criminal background checks, drug screen, and meet insurance requirements to drive company vehicles. Full time position with benefits at a non-profit agency with 40 years of history serving Southwest Louisiana.

    GOAL: Provide parenting education in group and in-home settings with families using models that focus on providing families with nurturing and parenting skills in order to prevent removal of the child from the home, abuse/neglect, or child welfare, as well as possible reunification.

    DUTIES AND RESPONSIBILITIES

    • Provide direct service hours for family.
    • Family Educator will provide approximately 15 hours of direct service contacts per week. A caseload averages 8 to 12 families. Other hours include documentation, group facilitation, traveling, staffing meetings, training, etc.
    • Services including mentoring, educating, possible transporting for special appointments.
    • 80% of the services are provided in the community
    • Observe and document progress of family in terms of the Family Service Delivery Plan (FSDP)
    • For the Nurturing Parenting curriculum services occur over a 17-20 week period with a possible extension with approval of the treatment team. For the Strengthening Families curriculum services occur over 6 – 8 weeks.
    • Visit Coaching is a supportive role in assisting with parenting education through modeling, teaching, and debriefing.
    • A cellular telephone is provided for communications. Use of the telephone is for emergencies and urgent client services.
    • Maintain a positive and therapeutic rapport with clients/families of the program
    • Attend all scheduled training and participate in providing training as required (at least 20 hours annually)
    • Attend scheduled staff meetings with Clinical Supervisor to discuss client status and goals.
    • Will serve Allen, Beauregard, Calcasieu, Cameron, and Jefferson Davis parishes

    II. DOCUMENTATION AND RECORDS

    • Maintain organized files and thorough documentation on each case in accordance with contract regulations. A description of each family contact and summary of services will be provided
    • Personnel documentation includes mileage, payroll, and family time records. Mileage is compensated at the rate of $.44 per mile.
    • Monthly Service Reports and Closure of Services form for cases are completed on each case. Final copies are submitted to the referral source.
    • Initial Assessments will be completed on each case assigned.
    • For Nurturing Parenting curriculum pre and post Competency tests, and pre and post Adult-Adolescent Parenting Inventory is to be completed with each client.
    • A Termination Summary of services will be submitted to the referral source
    • Weekly Progress Reports will be completed each week
    • III ESSENTIAL FUNCTIONS
    • Must possess a valid Louisiana chauffeur’s license and personal insurance coverage in order to transport a client
    • The driving record should not indicate repetitive offenses that may be considered a risk to personnel or equipment
    • Dependable personal transportation is necessary
    • Flexible hours for a non-traditional work schedule are necessary
    • The ability to lift and carry multi-media equipment in order to deliver educational services within the community
    • A criminal background check must be free of any charges that may represent a risk to the staff, agency, or population served
    • Proficient and effective communication skills and public speaking engagements are required
    • Ability to communicate effectively using clinical terminology in staffings, progress notes, group summaries, letters, and reports.
    • Ability to effectively present information and respond to verbal and written questions from co-workers, clients, referral sources, and the public.
    • Ability to add, subtract, multiply, and divide.
    • Ability to tolerate moderate noise levels.
    • Ability to drive up to 200 or more miles at a time
    • Ability to meet the physical demands as described:

    Sit - Over ⅔ of the time - Climb stairs - Under ⅓ of the time - Talk or hear - Over ⅔ of the time
