Position Overview: We are looking for a caring and dedicated LVN (Licensed Vocational Nurse) or RN (Registered Nurse) to serve as our Client Coordinator. The Client Coordinator will play a crucial role in ensuring a seamless transition for clients into our lines of business, providing personalized attention, support, and guidance every step of the way. This role requires strong clinical skills, excellent communication abilities, and a passion for delivering exceptional customer service to our clients and their families.
Key Responsibilities:
- Conduct initial assessments of clients referred for PAS or Nursing services, focusing on understanding their unique needs, preferences, and goals for independent living.
- Build trusting relationships with clients and their families, providing compassionate support, answering questions, and addressing concerns throughout the intake process.
- Collaborate closely with clients, their families, and healthcare professionals to develop customized care plans that prioritize their comfort, dignity, and independence.
- Coordinate the scheduling of PAS and/ or Nursing services, ensuring that clients receive the support they need when they need it, with a focus on responsiveness and flexibility.
- Serve as a resource and advocate for clients and their families, providing information, education, and guidance on navigating the home care system and accessing community resources.
- Ensure compliance with regulatory requirements, documentation standards, and quality assurance protocols, maintaining accurate and up-to-date client records.
- Participate in regular communication with clients, families, and caregivers to monitor satisfaction, address concerns, and adjust care plans as needed.
Qualifications:
- Current LVN license in good standing with the state board of nursing.
- Minimum of 2 years of clinical experience, preferably in home health or a related healthcare setting.
- Strong interpersonal skills and a genuine commitment to delivering exceptional customer service.
- Excellent communication skills, with the ability to empathetically engage with clients and their families, listen to their needs, and respond effectively.
- Proficiency in computer skills and electronic health records (EHR) systems.
- Ability to work collaboratively as part of a multidisciplinary team, prioritizing client-centered care and advocacy.
- Ability to perform skilled nursing visits and use clinical judgement to ensure the client is serviced by the most appropriate services.
