Clinical Facilities Planner offers professional services on assigned projects concerning design, construction, development, or installation of hospital buildings and facilities. Oversees construction progress, ensuring that timing, costs and contract requirements are adhered to. Being a Clinical Facilities Planner supervises construction, equipment installation, renovation and redesigning projects. Works with hospital departments to review design changes, equipment specifications/substitutions, purchasing and accounting procedures. Additionally, Clinical Facilities Planner develops and designs feasibility studies for review. May require a bachelor's degree. Typically reports to a manager or head of a unit/department. The Clinical Facilities Planner contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     As a Discharge Planner, you will work as part of a team to create and evaluate the discharge of clients from our treatment programs in a timely and efficient manner.
     

    Requirements:

    • Bachelor’s Degree from an accredited college or university, in related field; applicable experience may be substituted for a degree.
    • 3-5 years clinical case management/discharge planning experience
    • Strong interpersonal skills, excellent communication skills (both verbal & written)

    Responsibilities:

    • Provides appropriate and timely discharge planning from the facility, working closely with Utilization Review staff as to maximizing reimbursement and maintaining appropriate census at each level of care.
    • Responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return clients to referral source at time of discharge.
    • Provides written discharge planning to patients and families, determining which facility or community resources should be included in the patient's discharge plan, and requesting the determination of level of care for placement.
    • Accurately assesses patient's financial resources, including insurance benefits, and coordinates transitions to providers or other referral sources as deemed appropriate.
    • Documents discharge plan in EHR (Electronic Health Record), completing final documentation within two working days of discharge.
    • Functions as a member of a multidisciplinary treatment management team and the clinical department.
    • Provide exceptional customer service to all internal/external customers with rapid response to inquiries and concerns & support facility internal and external customer service standards.
    • Other responsibilities as assigned.
