Responsibilities
The Clinical Informatics Specialist will report directly to the Population Health Informatics Supervisor and be an integral part of the Population Health and Accountable Care Team. This role will participate in projects of critical and strategic importance to the organization, collaborating with key business, technical and clinical staff to plan and deploy enterprise data assets in support of these projects. This includes requirements gathering, data retrieval from a variety of clinical and financial data sources, validation, curation, and translation of data into informative and actionable visualizations. The position supports population health efforts to redesign care pathways, care gap closure outcomes, and process reporting dashboards. This role is responsible for tracking, trending, monitoring, reporting, and communicating results in multiple external and internal environments to improve the quality and efficiency of care delivered for our patients. UC Davis Population Health team evaluates clinical, financial, and environmental factors to identify and address barriers to optimal health for our attributed patients, identifies and prioritizes improvement opportunities, and implements and evaluates efficacy of system efforts.
Candidate should have the ability to travel to all clinics both on campus (UCDH) and to all the Community Physician practices (greater Sacramento area).
