The Clinical Informatics Coordinator work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project.   • CSO is looking for a Clinical Care Coordinator for our team at Friends of the Homeless in Northampton. 

    The Clinical Care Coordinator conducts biopsychosocial assessments and Individual Service Plan development with the client and the Case Manager and will coordinate substance abuse and mental health treatment services, case management, supports and projects activities including training, communication, referrals, and integrated services (e.g. dual diagnosis). 

    Responsibilities

    • Comprehensive biopsychosocial assessment inclusive of the outcome measures and other tools as determined necessary.
    • Early and ongoing team collaboration with case managers and outreach workers to ensure program participants' needs are identified and coordinated.
    • Development and implementation of an individual service plan (ISP) in collaboration with the individual collaterals.
    • Development of a risk management/safety plan in collaboration with the individual and collaterals.
    • Facilitation of care planning team (CPT) meetings.
    • Referrals and linkages to appropriate services along the continuum of care.
    • Identification and development of natural supports.
    • Substance abuse and Behavioral health education, advocacy and support.
    • Provide therapeutic support and services for uninsured and underinsured individuals.
    • Provide on-site psychoeducational groups related to specific BH/SA diagnoses and issues.
    • Provide consultation to the case managers and outreach workers regarding the individual's behavioral health and/or addiction treatment needs.
    • Be available to work flexible schedule which may include evening and weekend work, as required by the needs of families receiving services.

    Qualifications

    Master's degree in a behavioral health discipline or related field, and applicable experience. License preferred. Minimum one year demonstrated experience working with identified population. Knowledge of and/or training in working with identified population and related evidence based interventions. Good listening skills; ability to establish trusting relationships through compassion, empathy and insight, and ability to foster self-sufficiency in families using problem-solving and practical skills; familiarity with support groups. Excellent oral and written communication skills and computer proficiency required including ability to work with email, electronic health records and word processing software. Must be able to work independently and follow through with projects as well as be an effective member of a team. Must be able to work flexible schedule which may include evening and weekend work, as required by clients' needs. Valid driver's license and reliable transportation required.
