Ensures effective coordination of care among the team. Participates in care conferences and care coordination meetings to assure the best possible outcomes for patients, appropriate length of stay, goal achievement, and effective discharge planning.
Guides complex clinical care to assure compliance with policy, procedures, and established standards of care. Promotes timely, proactive interventions to effectively mitigate potential decline in patient condition
