    You will be providing oversight to the agencies clinical activities by clinical staff with a focus on our direct care workers. You will partner with other Clinical Managers to ensure overall agency clinical compliance and Iniatives are met.

    Responsible for creating a client-focused, high- performance, high-commitment department by planning, developing, implementing, coordinating and evaluating clinical activities throughout the agency

    Ensures effective coordination of care among the team. Participates in care conferences and care coordination meetings to assure the best possible outcomes for patients, appropriate length of stay, goal achievement, and effective discharge planning.

    Guides complex clinical care to assure compliance with policy, procedures, and established standards of care. Promotes timely, proactive interventions to effectively mitigate potential decline in patient condition

    Requirements:

    • RN licensed in Iowa
    • 2 years previous Home Care Experience
    • 2 years previous Clinical Leadership
