Clinical Nurse Specialist - Home Care assesses a homecare facility's nursing practice and its impact on patient outcomes. Our mission is to enhance the lives of aging adults and their families. Our service areas are Augusta, Rockingham, and Rockbridge counties. Our Staff Nurse works from our Verona office. A company car and gas card are provided for home visits. 

    Our Staff Nurse:
    • Completes initial personal care assessments and develops a written plan of care with the client/family member
    • Re-evaluates the client's care needs every 90 days and prepares progress notes on the client's personal care services
    • Teaches classes in non-medical personal care services to our onboarding caregivers
    • Participates in RN on-call rotation by being available to answer questions from caregivers and clients/families after 5:00 p.m. on weekends and on corresponding weekends

    Qualifications:
    • Associate Degree in Nursing 
    • Registered Nurse (RN) active in Virginia

    • Speak and write English at a competent level
    • Computer skills
    • Ability to participate in RN on-call rotation
