Clinical Nurse Specialist - Home Care jobs in Paramus, NJ

We are seeking a compassionate and skilled Home Care Nurse to provide high-quality care to patients in their homes. Duties:
- Provide comprehensive nursing care to patients in their homes
- Conduct assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and response to treatment
- Assist with activities of daily living, including personal hygiene and mobility
- Educate patients and their families on disease management and self-care techniques
- Coordinate with other healthcare professionals to ensure continuity of care
- Conduct Performance Appraisal, in-services and supervise CHHA
- Assist with discharge planning and coordination of follow-up care

Qualifications:
- Valid nursing license in the state of New Jersey
- Minimum [2] years of nursing experience, preferably in home care or a related field
- Proficient in eClinicalWorks or similar electronic medical record system
- Strong knowledge of primary care principles and practices
- Experience in Home Health Care, laws, policies and procedures
- Excellent communication and interpersonal skills
- Ability to work independently and prioritize tasks effectively
- Knowledge of coding and billing procedures is a plus
