Clinical Nurse Specialist - Home Care assesses a homecare facility's nursing practice and its impact on patient outcomes. Provides clinical direction to nursing staff in the delivery of patient care programs. Being a Clinical Nurse Specialist - Home Care ensures nursing staff maintains an established level of clinical competency. Requires a master's degree. Additionally, Clinical Nurse Specialist - Home Care typically reports to a manager or head of a unit/department. Requires certification as a registered nurse (RN). The Clinical Nurse Specialist - Home Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes. To be a Clinical Nurse Specialist - Home Care typically requires 5 years experience in the related area as an individual contributor. 1 to 3 years supervisory experience may be required.     We’re hiring a Clinical Care Specialist to join our Community Access Recovery and Engagement (CARE) team at 41 Montebello Road in Pueblo.


    What You’d Be Doing


    As the Clinical Care Specialist you would play a central role in helping our clients’ access services and take the next steps to wellness. Your work would take you to homes, and other settings, and call upon you to collaborate with other professionals within and outside our agency to assure that clients receive the very best care available.


    Specifically, the Clinical Care Specialist


    • Maintains a caseload and conducts case management services for clinicians;
    • Makes appropriate referrals to address mental health and substance use needs;
    • Links clients and community resources effectively;
    • Offers behavioral health counseling and outreach;
    • Ensures clients attend scheduled appointments in the community and at Health Solutions;
    • Identifies stressors and precursors that led to the current situation or crises;
    • Identifies available coping mechanisms and natural supports;
    • Enhances problem-solving, independent living, and socialization skills among clients;
    • Provides support for clients to remain in home and community to avoid more restrictive living arrangements;
    • Maintains accurate records in the agency’s electronic health record; and
    • Performs other job-related duties assigned by the director or designee.
    • Physical requirements include the following:
    • Must be able to remain in a stationary position at least 50% of the time.
    • Frequently communicates with physicians about medical notes, must be able to exchange accurate information in these situations.
    • Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
    • Occasionally lifts and carries items up to 20 pounds
    • Occasional exposure to communicable diseases and bodily fluids

    What We’re Looking For—The Must-Haves

    • Bachelor’s degree in social work, psychology, or a closely related field

    • Colorado driver’s license, a good driving record, and access to transportation during work hours

    • Experience working with adults with severe and persistent mental illness
