Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care. This position promotes and supports the Company's core values of Compassion, a positive Attitude, Respect and Excellence.

We are looking for a compassionate, detail-oriented Clinical Manager to join our team. The Clinical Manager is responsible for monitoring, coordinating, and managing all aspects of patient clinical care administered by the field clinicians. The Clinical Manager will effectively interact with patients, caregivers, family members and provide leadership to field clinicians to ensure quality care on all levels.

Required Qualifications:

• Must possess an active Virginia RN license - REQUIRED.
• Minimum of 3 years Home Health experience - REQUIRED.
• Previous experience with Homecare Homebase EMR system - PREFERRED.
• 2 years in a Clinical Supervisory role or management level position
• Minimum of Bachelor's degree, or LPN with five years of experience
• Ability to apply and analyze data as it pertains to quality and compliance metrics.
• Comfortable working collaboratively in cross-functional teams
