Clinical Outcomes Director directs in-depth analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees and leads staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Evaluates patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Position Title: Residential Site Supervisor (RSS)
SUMMARY:
The Personal Outcomes Manager assists the Residential Director in the operation of the adult Residential Services program which includes support of individuals in multiple locations such as assisted living homes, supervised apartments, independent apartments and homes. The position has primary responsibility for: Ensuring that the department’s provision of services comply with all applicable regulations, standards and internal procedures.
The position is also responsible for assisting the Residential Director with program planning and development, budget and expenditure monitoring, staff recruitment, training and supervision, individual intake process, implementation of Personal Services Plans (PSP’s) and interagency coordination. In the absence of the Residential Director, the Personal Outcomes Manager may assume the functions of that position.
