Clinical Outcomes Director directs in-depth analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees and leads staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a Clinical Outcomes Director creates, defines, and implements performance metrics to review program effectiveness. Evaluates patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, Clinical Outcomes Director identifies opportunities and provides recommendations to improve program delivery, patient outcomes, and operational efficiency. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to senior management. The Clinical Outcomes Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Clinical Outcomes Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Job Duties

    The Clinical Director at GSCH will supervise all clinical operations across our network of Federally Qualified Health Centers (FQHC) providing integrated Primary Care and other care services to a vulnerable patient population. In partnership with the Chief Medical Officer (CMO), the Chief Operating Officer (COO) and the Director of Quality Improvement and Population Health, the Clinical Director will oversee and manage delivery of Primary Care, Prenatal Care, Pediatric Care, and Medication Assisted Recovery (MAR) Care services across all clinical sites and programs. Clinic sites include Goodwin Community Health in Somersworth, NH; Families First Health & Support Center in Portsmouth, NH; the School-Based Clinic at Somersworth High School in Somersworth, NH. Clinical programs include the Mobile Health Program, the Tobacco Cessation Program, the Remote Patient Monitoring Program, the Breast and Cervical Cancer Program (BCCP), and more.The Clinical Director will also ensure compliance with federal Patient Centered Medical Home accreditation. Ideal candidates for the Clinical Director position are RNs with prior managerial experience excited to work with multiple shareholders in an FQHC setting to improve patient access to excellent care in an integrated, community health setting.


    At Greater Seacoast, the Clinical Director will manage the following:

    • partners with the CMO and clinical management to develop and implement best clinical practices
    • partners with the CMO to manage and improve team-based care delivery of services
    • partners with the Director of QI and Population Health to identify quality of care indicators and improve performance against indicators
    • develops new clinical programming under direction of the COO
    • partners with Site Directors and other clinical management to optimize and manage clinical workflows across all clinic sites
    • take initiative to build internal and external partnerships
    • work with Leadership to uphold agency values
    • manage the Patient Centered Medical Home accreditation due diligence processes
    • represents Clinical Operations at various internal and external committees including the CQI Committee, the Patient Experience Committee, CHAN user group, ProHealth user group
    • ensures that programs are in compliance with HRSA, FTCA, State and other applicable agency requirements
    • responsible for assessing the training needs of the clinical staff and developing a training schedule annual for the clinical staff


    Qualifications

    • Education: RN Degree required; Master's and/or MBA degree preferred, but not required
    • License: Active NH or compact state RN license
    • Experience: Minimum 3 years’ managerial experience in a clinical setting, preferably in a multi-site environment
    • Other: Current Basic Life Support (BLS) certification required
