CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     Position Title: Residential Site Supervisor (RSS)

    SUMMARY:

    The Personal Outcomes Manager assists the Residential Director in the operation of the adult Residential Services program which includes support of individuals in multiple locations such as assisted living homes, supervised apartments, independent apartments and homes. The position has primary responsibility for:  Ensuring that the department’s provision of services comply with all applicable regulations, standards and internal procedures. 

    The position is also responsible for assisting the Residential Director with program planning and development, budget and expenditure monitoring, staff recruitment, training and supervision, individual intake process, implementation of Personal Services Plans (PSP’s) and interagency coordination.   In the absence of the Residential Director, the Personal Outcomes Manager may assume the functions of that position. 

     

     

    MINIMUM EDUCATION AND EXPERIENCE:

    • Bachelor’s Degree in Social or Behavioral Sciences, Education, Rehabilitation, Business/Health Administration or related field.
    • Three years management experience.
    • Three years supervisory experience in a residential program for individuals with developmental disabilities, preferred.
    • Two years’ experience working with individuals experiencing developmental disabilities.
    • Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).

     

    KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

    • An organized and detail-oriented manager, able to prioritize and delegate tasks effectively to ensure timely completion within a team environment.
    • Able to communicate effectively with all organizational levels, build relationships of trust and enlist the participation of team members.
    • Able to work independently and effectively coordinate tasks to accomplish projects with timeliness and creativity.
    • Thorough understanding of and personal commitment to least restrictive and individualized support for individuals. 
    • Demonstrated proficiency in direct care activities and training, and in monitoring the physical, emotional and health needs of the individual receiving support.
    • Thorough knowledge of residential services, applicable policies and regulations, and experience with individualized service planning.
    • Must have good writing skills and computer experience with word processing, spreadsheets and electronic medical records.
    • Must be able to maintain flexible hours, including on-call for evenings and weekends
    • Must have reliable transportation.

     

     

    LICENSES/CERTIFICATIONS REQUIRED: 

    • Alaska Driver’s License

    IMMUNIZATION REQUIRED:

    • Annual PPD Test
    • Hepatitis B immunization (or) Waiver
    • Covid 19 immunization or medical/religious exemption

     

    WORKING CONDITIONS:

    • Sedentary Work: Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.  Sedentary work involves sitting most of the time.  Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
    • Light Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force constantly to move objects.  

     

    PHYSICAL ACTIVITIES:

    • Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
    • Talking: Expressing or exchanging ideas by means of spoken word. Activities demand detailed and important instructions spoken to other workers accurately, loudly or quickly.
    • Driving: Minimum standards required by State Law (including license).
    • Repetitive Motions:  Substantial movements (motions) of the wrist, hands, and/or fingers.

    None: The worker is not substantially exposed to adverse environmental conditions.
