CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience. Better Birth Outcomes (BBO) Case Manager - Part-Time

    Minimum Qualifications: Registered professional nurse license pursuant to Section 12 of the Illinois Nursing Act of 1987, Certification of Graduation from a school of nursing approved by the Department of Professional Regulation and a current Illinois license as a Registered Nurse (R.N.) by examination, or Masters of Social Work (MSW) licensed pursuant to Section 9A of the Clinical Social Work and Social Work Practice Act; Masters in Counseling Psychology, Licensed Professional Counselor (LPC); Licensed Clinical Professional Counselor (LCPC), Masters in Human Services and Counseling with specialization in counseling, Masters in Public Health (MPH) with specialization in public health nursing, women’s health, or reproductive health. R.N. must submit proof of 20 Continuing Education Units (CEUs) every license period; copy of license; and copy of CPR/AED card. MSW must submit proof of CEUs and copy of license every license period.

    Must possess the ability to establish and maintain positive relationships with customers, providers, the general public, and other professional personnel. Must have the ability to work with diverse community groups. Must have a dependable vehicle for use during work hours, valid Illinois Driver’s License, and appropriate auto insurance. Must be physically able to get in and out of car many times per day in all types of weather and be able to adapt in a positive manner to various client living conditions (i.e., dirty, cramped). Must be able to carry, pull, and manage a minimum of 30 pounds of equipment daily.

    Summary of Job Duties: Provides case management with integrated WIC services to eligible clients with regard to local, state, and federal policies and procedures. Completes an appropriate number of face-to-face or home visits and prepares a care plan based on the assessment of the whole family. If assigned, follows appropriate policies and procedures for the following programs: Family Case Management, Healthworks Lead Agency, High Risk Infant Follow-up and WIC. Completes BBO outreach process, if assigned. Delivers curriculum per program requirements. Follows appropriate policies and procedures for the BBO Program. Maintains current knowledge of community resources related to referral needs. Completes documentation per protocol utilizing the Illinois Department of Health and Human Services data entry system. Exhibits competence with multicultural awareness and resulting effect on nutrition and health practices/problems. Attends meetings, trainings, and events as determined by the WIC/FCM Director and Assistant Director.
