CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     Description:

    Hospice nurses are registered nurses who care for terminally ill patients. They develop care plans for individual patients, monitor patients' conditions, and provide guidance and support to patients and their families. They work in private homes, residential care facilities, nursing centers, and other hospice care environments.

    • Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
    • Preparing and adjusting patient care plans as needed.
    • Frequently visiting patients to monitor vital signs and physical comfort levels.
    • Performing prescribed treatments and administering pain management medications.
    • Informing physicians about changes in patients' conditions.
