CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.   • Director of Clinical Services/Clinical Manager – full-time position available in busy home health/ hospice/ private duty agency with the opportunity to work with excellent staff and community partners. This individual will provide clinical coordination and supervision to ensure appropriate care and quality of service across/among Agency clinical programs and provides overall planning, direction, coordination, management and supervision of all nursing and professional services.

    Qualifications:

    • Must be Registered Nurse, Physical Therapists, Speech-Language Pathologist, Occupational Therapist or Social Worker with suitable operational experience, in good standing and able to meet all federal and state licensure requirements
    • RN with Bachelor of Science in Nursing preferred
    • Minimum of five years’ experience in home health, public health, or health administration and progressive leadership/management responsibility in health care.

    DUTIES & RESPONSIBILITIES

    • Oversee the professional and home health aide delivery of services of the agency to ensure service quality
    • Manage operational budgets with Chief Executive Officer and Director of Finance ensuring revenue opportunities are maximized with effective and efficient operational practices
    • Facilitate QAPI Plan in collaboration with the Director of Operational Excellence and participate in quality improvement (QI) activities
    • Keep informed and up to date in all aspects of home care and management through literature, workshops, seminars, courses and conferences and promotes staff development
    • Oversee and support the coordination of patient care services. Attends and participates in patient care conferences
    • Ensure an ongoing quality assurance program that meets the following requirements: case assignment and management is appropriate, adequate, and consistent with the plan of care, medical regimen and patient needs; all services provided to the patient are coordinated, appropriate, adequate and consistent with plans of care; and findings of the quality assurance program are used to improve services
    • Provide guidance and is a resource for internal and external clinical staff
    • Ensure clinical records are completed timely and effectively for compliance
    • Ensure compliance with State and Federal regulations/guidelines and clinical practice standards
    • Participate in agency strategic planning; partners with Chief Executive Officer and Management Team on clinical program(s) growth and development
    • Recruit, interview, hire, orient and supervise staff
    • Ensure that all staff is adequately oriented to the Agency prior to patient contact
    • Assist in developing and facilitating in-service education
    • Assist with revisions and updates to job descriptions and performance standards
