CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     A DAY IN THE LIFE OF OUR HOSPICE EDUCATION AND OUTCOMES COODINATOR (RN)

    As our Hospice Education and Clinical Outcomes Coordinator, you arrive each day ready to take our hospice services above and beyond. The primary function is to assume responsibility for administrative tasks and leadership functions for assigned office(s) in coordination with Administrator. You manage our office and clinical operations in order to meet the needs of our patients and help them live each day to the fullest. Ensuring our patient's needs are met, you coordinate the daily workload and 24-hour services. Working with the HR department, you assist with recruiting, hiring, and training new employees. You act as a resource for our field staff, investigate and resolve all reported incidents, conduct performance evaluations, and develop performance improvement plans as well as educate employees on infection control and how to recognize and report abuse. Other duties include monitoring the KPI, the QAPI program, and compliance. You will also be responsible for reviewing HIPAA complaints and work with CM and Administrator to help resolve the complaints.

    QUALIFICATIONS

    • Oklahoma RN license
    • 4 years of nursing experience
    • 2 year of hospice leadership roles.
    • Ability to obtain CPR certification within 60 days of hire.
    • Knowledge about the Medicare guidelines governing Hospice agencies.
    • Valid Oklahoma driver's license and vehicle insurance
    • Basic computer skills

    WORK SCHEDULE
