CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience. Substance Use Counselor- Clinical Case Manager

  • Parenting Awareness and Drug Risk Education Services (PADRES) Clinical Case Manager

    Job Duties:

    • Offer PADRES services to individuals and families in need of help with substance misuse issues in Abilene and the surrounding counties. The target population for services includes individuals who are pregnant or have children under six years of age, who are involved with the Department of Family and Protective Services (DFPS), have a history of substance use, or are at risk for substance use. PADRES services include, but are not limited to: substance use screenings, assessments, referrals for recovery support services, parenting skills training and education, Fetal Alcohol Spectrum Disorder (FASD) education, pregnancy and STD/HIV testing, reproductive health and STD/HIV education, communication and conflict resolution skills training, trauma-informed services, service planning, case management, motivational counseling, home visits, transportation assistance, and follow-ups.
    • Additionally, the Clinical Case Manager will serve as a liaison and advocate for program participants who are involved with DFPS by participating in conferences and/or attending court dates.
    • Provide accurate information for client entries and case management for client referrals.
    • Attend community-coordination meetings and act as a liaison with other concerned individuals and community organizations in order to address substance use issues.
    • Attend training specific to the program’s responsibilities and continuing education requirements for maintaining counseling credentials (All training costs are paid by Abilene Recovery Council).
    • Maintain effective record keeping, ensuring that all files are complete, kept in good order, and secured to protect client confidentiality.
    • Strive toward the highest quality and standards of professional conduct.
    • Provide informational presentations relating to substance misuse, reproductive health, parenting, STD/STIs, and recovery services.

    Minimum Qualifications:

    • High school graduate or GED
    • College or work experience equivalent
    • REQUIRED-- Must be a Licensed Chemical Dependency Counselor (LCDC) or LCDC-Intern

    Required Knowledge, Skills, and Abilities:

    • Ability to work independently and communicate effectively- orally and in writing
    • Ability to maintain effective working relationships with clients and agencies
    • Ability to maintain current knowledge of prevention and intervention techniques; information on alcohol, tobacco, and other drugs use, misuse, and addiction; and available programs and services
    • Ability to travel to outreach locations in a reliable vehicle 1-2 times per week
    • Ability to maintain confidentiality of clients and their families/partners
    • Ability to occasionally work 1-2 evenings per week and on the weekends (staff utilize flex time to maintain a 40-hour work week)
    • Ability to maintain required credentials

    Benefits:
