Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh).     JOB PURPOSE OR MISSION: Fills orders for drugs, monitors patient drug therapies and provides drug information; supervises and directs supportive personnel. Job is performed appropriately for the age population served, as defined in the department’s scope of service.

    JOB FUNCTIONS

    ESSENTIAL JOB FUNCTIONS include, but are not limited to:

    1. Prepares and dispenses drug orders per physician request according to established policies, procedures and protocols.

    PERFORMANCE STANDARDS: • Interprets drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately. • Reviews and verifies Computer Physician Order Entry medications and contacts physicians when questions arise. • Maintains accurate, complete pharmacy patient medication profiles. • Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately in accordance with USP . • Issues controlled substances to patient care areas and maintains records as required by law.

    2. Ensures safe, appropriate, cost effective drug therapies for patients according to established policies, procedures and protocols.

    PERFORMANCE STANDARDS • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. • Reads, extracts and interprets information in patient charts appropriately. • Detects and reports suspected adverse drug reactions accurately and in a timely manner. • Provides clinical consultation and clarification to practitioners, suggests appropriate cost-effective therapeutic alternatives to medical staff as needed. • Documents all clinical activities and interventions accurately and completely. • Participates in the quality improvement and medication use review activities of the department; collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.

    3. Contributes to the quality and effective operation of the department through supervision of pharmacy support personnel.

    PERFORMANCE STANDARDS • Verifies the daily activities of pharmacy technicians and participates in performance appraisal of pharmacy support personnel. • Ensures pharmacy services are provided in a timely manner through organization and prioritizing work assignments; works independently with minimal supervision. • Completes all assigned medication storage area inspections in a timely manner and identifies and replaces outdated and unusable drugs. • Professionally answers the telephone and requests at the window.

    4. Help to ensure compliance with accreditation standards and with federal, state, and local regulations related the practice of pharmacy. PERFORMANCE STANDARDS: • Manage routine audits for narcotic documentation, adverse drug events, or other problem identification. • Coordinate compliance-related issues in medication areas including information/data management, order entry process and smart system technology.

    5. Responsible for the development and use of technology and automation used in providing quality pharmaceutical care services.

    PERFORMANCE STANDARDS: • Assists the department director, department managers, and staff by providing data derived from automation technology in order to make sound operational decisions. • Serves as a liaison to technology vendors (ex: McKesson) IS department, Billing and Accounting, Clinical Informatics, Nursing, and other departments as required. • Actively participates in pharmacy and nursing orientation, automation training and management of automation ID access. • Participates in functional testing before and after system upgrades and improvements.

    6. Maintains competency required for current job title/position

    PERFORMANCE STANDARDS: • Maintains current pharmacist license; ensures applicable CE records and licensure are maintained in department files. • Attends pharmacy staff meetings or read and signs acknowledgement of the minutes. • Attends orientation, education and training programs; reviews literature and other materials pertinent to the practice of pharmacy. • Completes all competency/skills assessment requirements.

    7. Performs all other duties as assigned. • Willingly participates in Work Excellence initiatives

    HIPAA REQUIREMENTS: Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to Job Position including but not limited to: Medical records without limitation both paper and electronic, patient demographics, lab and radiology results, patient information related to surgery or appointment schedules, medical records related to quality/data, patient financial information and 3rd party billing, information related to patient location, religious beliefs, research information, public health records, employee prescriptions, and/or investigatory information.

    SAFETY REQUIREMENTS: Maintains knowledge of and adherence to all applicable safety practices appropriate to Job Position including but not limited to: Incident reporting, handling of wastes, sharps and linen, PPE, exposure control plans, hand washing, environment of care, patient identification, receives meds and/or orders.

    PHYSICAL DEMANDS (Reference DOT 074.161-010 – PHARMACIST) - STRENGTH: L – Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
