  • Nemours is seeking a Clinical Pharmacist (evening shift) to join our Nemours Children's Health team in Wilmington, Delaware.

    Must be licensed or license eligible in DE, PA, & NJ.

    The Clinical Pharmacist is responsible for providing pharmacy services from the Central Pharmacy and/or Outpatient Pharmacies. The Clinical Pharmacist is responsible for reviewing, clarifying, and making recommendations for all medication orders, and responsible for coordinating & checking all the activities of the Certified Pharmacy Technicians. This Pharmacist will provide age and developmentally appropriate care to infants, children, adolescents, and adults. They will also provide drug information & education to hospital personnel and community, as well as assist with quality improvement initiatives. Ensures compliance with State & Federal regulations concerning Controlled Substances.

    • Previews and interprets medications orders for accuracy using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.
    • Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians and practitioners on issues concerning drug therapy, the inherent toxicity of drugs, drug-drug, drug-food interactions, and side effects, as well as assisting in the prescribing of appropriate doses.
    • Responsible to provide accurate, adequate, and timely drug information to the professional staff and to patients and their families.
    • Coordinates the activities of assigned certified pharmacy technicians engaged in the preparation, labeling and delivery of medications in accordance with federal, state, and local laws and regulations pertaining to the dispensing of drugs, controlled substance medications, investigational drugs, and the maintenance of required records.
    • Facilitates orientations and educational programs for the medical staff and residents regarding the appropriate utilization of Pharmacy Services.
    • Participates in medication use review activities of the department. Collects data; maintain logs, records, and other documentation as assigned.
    • Reads, extracts, and interprets information in patient’s charts accurately.
    • Promotes the use of the Nemours/AIDHC drug formulary by converting non-formulary orders to formulary when possible and coordinates procurement of non-formulary dugs when necessary.
    • Mentors pharmacists-in-training including, but not limited to, Pharmacy Practice Residents, School of Pharmacy clerkship students; activities include both practical and didactic experiences.
    • Maintains pharmacy facilities and equipment, including automated dispensing equipment:
      • Follow policies & procedures for sanitation management and hazardous waste handling. Follow safety policies and procedures when disposing of hazardous and non-hazardous wastes.
      • Apply appropriate techniques to clean laminar flow biological safety cabinets.
      • Demonstrate skill in cleaning up a cytotoxic or other hazardous medication product spill using the accepted procedure.
      • Maintain clean and neat work equipment.
      • Accurately calibrate weighing or counting devices, fluid compounders, or syringe pumps.
      • Compound products using appropriate techniques, equipment and devices.
      • Follow manufacturer’s guidelines in troubleshooting, maintaining, and repairing electronic devices used in preparing and dispensing medications.
      • Demonstrate the ability to use technologies frequently used in the pharmacy setting, including bar coding and automated dispensing technology, and unit dose packaging & reporting.
    • Inventory Control:
      • Kanban Inventory system is adhered to 100% of time.
      • Maintains and organizes inventory supply in a manner, which minimizes inventory storage cost, wastage, and stock- outs. Ensures inventory is rotated properly, stored in appropriate conditions and location with 100% accuracy.
      • Completes monthly medication expiration dating assignments in a timely manner. Identifies and reorders expired medication inventory and ensures that stock is stored in accordance with manufacturer requirements.
      • Assures the proper replacement of prescription drugs to stock upon requisitions from various departments of the hospital either manually or through the Pyxis System.
      • Ensures the secure storage of controlled substances.
      • Documents every transaction completely and accurately.
      • No generation of discrepancies in the CII Safe, Pyxis MedStation, A-Systems or Inventory Logs.
      • Ensures the destruction of returned controlled substance continuous infusions.
      • Ensures secure storage of controlled substances on a regular basis. Maintains inventory records of controlled substances and other drugs as required by law.
      • Performs inventory counts of controlled substances as per policy and procedures.
    • Participates in hospital and departmental Continuous Improvement (CI) activities:
      • Utilizes the Lean Principals to analyze and resolve problems in a systematic manner
      • All improvements must be made in accordance with the scientific method
      • Participation in departmental Rapid Process Improvement (RPI) activities
      • Provides coverage (overtime) to allow coworkers to participate in department RPI
      • Achieves patient safety and satisfaction through the use of lean principles:
        • 5S
          •    Elimination of Waste
          •    Just In Time (JIT)
          •    Standard Work
          •    Quality
      • Promote and Comply with Standardized Procedures.
      • Promote and Comply with Standard Work.
      • Identify “waste” in ownership process and recommend solutions for implementation.
      • Correct abnormal conditions.
    • Performs other related duties as assigned by authorized personnel to provide departmental flexibility to patient care needs including those for emergency situations.
      • Provides help or coverage to others when needed.
    • Achieve certification, such as a Basic Life Support (BLS) Healthcare Provider, and other required competencies per department policy.

    Job Requirements

    • B.S. in Pharmacy or PharmD required.
    • Minimum of one (1) to three (3) years experience required.
    • Must be licensed or license eligible in DE, PA, & NJ.  Associates who are not licensed in the state of DE at time of hire must start process for licensure and must obtain that DE licensure on or before the 90th day of employment.  Failure to do may result in suspension pending termination.
    • Associates who are not licensed in state(s) of PA, or NJ must obtain licensure for those states within 6 months after start date.  Failure to do so  may result in suspension pending termination.
    • Non Sterile operations (Unit dose training, etc.); Sterile operations (IV compounding, etc.).
    • TPN, Oncology and Dialysis experience; Drug Storage; Distribution, and Automation Technology Systems; Pharmacy Software.
    • Hospital Pharmacy experience preferred, Pediatric Pharmacy experience preferred.
