    Our Director of Clinical Services Will:

    • Oversee all clinical operations of the assigned hospice location including patient care, staffing, communication, regulatory compliance, compliance with budget, and marketing to ensure optimal performance and fiscal responsibility to include:
      • High-quality patient care: Ensure the organization provides excellent patient care as measured by patient/family feedback, CAHPS surveys, and community response/advice.
      • Strategic growth: Provide support in leading the organization to sustainable, ethical, and profitable growth as measured by goals for ADC, budgeting, and financial metrics.
      • Customer Service: Work to ensure meaningful and effective engagement and relationships with communities, families, and patients as measured by community utilization and feedback.
      • QAPI: Facilitate process improvement initiatives in customer relations, quality assurance, financial management, and staff development as measured by QAPI program development and survey outcomes (CMS, ACHC, or CHAP).
      • Culture: Foster a culture in alignment with the Heritage Core Values as measured by associate feedback and surveys.

    Our Clinical Coordinator Has:

    • Current licensure to practice professional nursing in the State.
    • A degree in Nursing from an accredited program by the National League for Nursing (Bachelor’s or Master’s degree is preferred).
    • The ability to work and communicate effectively with diverse groups including colleagues, clients, and community partners.
    • An in-depth understanding of hospice care and the services provided to patients, families, and caregivers through an interdisciplinary group
