    Pay Range : $68,000-$75,000  

    DUTIES AND RESPONSIBILITIES: 
    1. Assures direct care staff functions provide compliant care. 
    1. Provides training and guidance to agency direct care staff regarding orders for disciplines and/or treatments and services, consistent with agency policy 
    1. Ensure referrals result in a timely start of care or are well documented for permitted delays or non-admissions. 
    1. Effectively manages a caseload of clients. 
    1. Develops and implements appropriate treatment programs; develops realistic personal goals with patient; re-evaluates, sets new goals, adjusts care accordingly and evaluates for discharge potential in consultation with physician or intermediate care provider. 
    1. Completes all supervisory visits, comprehensive assessment/reassessment visits per Agency policy, federal and state rules, and documenting the visits. 
    1. Conducts coordination of care between direct care staff, and external providers. 
    1. Serves as liaison between this Agency and physicians, hospitals, and other agencies regarding patient care.  
    1. Provides positive attitude and example of professional behavior for direct care staff. 
    1. Provides orientation and competency verification of direct care staff.  
    1. Provides direct, in field, supervision of newly hired clinicians and assures supervision of all staff is completed per regulations and agency policy including documentation of same. 
    1. Provides direct response and guidance to staff for problem resolution in patient care. 
    1. Provides information and reports to Director of Clinical Services/Administrator as instructed.  
    1. Participates, as requested, in orientation of new staff, in-service education, continuing education, utilization review and QAPI programs. 
    1. Participates in/attends staff development activities and in-service meetings to educate the personnel 
    1. Provides training and in-services to agency direct care staff on the following, but not limited to, topics required by state licensure, clinical services, rules and regulations, agency policies and expectations. 
    1. Serves as a member of the administrative on-call team. Responsible to complete on-call logs and assure follow up completed and documented. 
    1. Participates, as assigned by Administrator, on compilation of data for the annual agency evaluation and/or quarterly governing body review reports 
     
    OTHER DUTIES:  Other duties as assigned. 
     
    EXPERIENCE REQUIRED: 
    1. Proven supervisory and communication skills. 
    1. Proven ability to act as liaison among staff and community. 
    1. Case Management/Supervisory experience preferred. 
     
    QUALIFICATIONS/EDUCATION REQUIRED: 
    1. Registered Nurse with current state licensure, and  
    2. Bachelor’s degree preferred, Associates degree considered. 
    3. Experience in healthcare of at least two (2) years with a minimum of two (2) years in home health preferred. 
     
    ABILITIES, SKILLS AND KNOWLEDGE:   
    Understanding of the Medicare and Medicaid regulatory guidelines and state licensure. 
    Expertise in coordinating work activities of multi-disciplinary team. 
    Excellent oral and written communications skills with proven ability to communicate with technical and 
      Non-technical users. 
    Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.   
    Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.   
    Ability to effectively present information to clients, employees and community members.   
    Ability to respond timely and effectively to the most sensitive inquiries or complaints.   
    Ability to define problems, collect data, establish facts, and draw valid conclusions.   
    Ability to send/receive emails and use the internet for research and informational purposes.   
    Ability to use word processing software for communication and documentation purposes.   
    Ability to use independent judgment to manage and impart confidential information. 
    Skill to attend to multiple tasks at once which are time sensitive. 
    Basic knowledge of Windows based operating systems, Electronic Health Records and ability to advance computer skills as required. 
    Ability to establish and maintain effective working relationships with fellow employees and the public.  
     
    SPECIAL REQUIREMENTS:  
    Current state RN licensure without stipulations 
    Current TB skin tests upon hire and screening annually thereafter;  
    Possession of a valid state Driver's License with reliable transportation and applicable car insurance.  
     
    PHYSICAL REQUIREMENTS:  
    Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with  hands and arms and talk and hear.   
    Ability to frequently use hands to finger, handle, or feel.   
    Ability to occasionally climb. 
    Ability to occasionally use sense of smell.   
    Ability to frequently lift and/or move up to 40 pounds. 
    Ability to use close vision, distance vision, color vision, peripheral vision, depth perception. 
    Ability to adjust focus. 
     
    WORK ENVIRONMENT:  
    The employee is frequently exposed to outside weather conditions.   
    The work environment is highly variable, as care is provided in private homes.   
    Lighting, acoustics, air quality, temperature and environmental hazards are not predictable.   
    The employee is occasionally exposed to wet and/or cold conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.   
    The noise level in the office work environment is usually moderate. 
     
    TRAVEL:  
    The employee is required to travel frequently to complete agency business
