    Job Summary: Provides clinical support and precepting to agency direct care staff to assure adherence to agency
    policies and federal and state regulations; serves as liaison between clinical staff and operations staff; and as
    liaison among the Agency, hospitals, the medical community, and general community. The Clinical Care Supervisor
    will also provide direct skilled, professional nursing care to clients as prescribed by the physician and in compliance
    as established by the State Board of Registered Nursing, the client, and the policies of Angels of Care.

    ESSENTIAL FUNCTIONS:
    1. Staff billable hours as assigned by supervisor with any client (week or weekend) for any driving distance
    less than or equal to 2 hours from your home.
    a. Any distance more than 2 hours will be included in your total paid hours.
    b. Any distances that exceed the distance from your home to the office will be paid per mileage
    policy.
    c. Maintain current CPR certification.
    d. Demonstrating pediatric competency, satisfactory observations, good nursing judgment, and
    communication skills.
    e. Working with little or no supervision.
    f. Assisting the client’s family and other health care personnel in providing continuity of care.
    g. Working in cooperation with the family and health care team members to meet the emotional
    needs of the client and family.
    h. Supervising ancillary personnel and delegating responsibilities when required.
    i. Accepting responsibility of assignment to perform specialized procedures, such as I.V. therapy
    only when qualified with specialty training, proven competency, clinical background, and
    expertise in the specialty area.
    j. Assuming responsibility for personal growth and developing, maintaining, and upgrading
    processional knowledge as well as practicing skills through attendance and participating in
    continuing education and in-service classes.
    k. Assisting the client with personal care along with basic nursing care.
    l. Performing all skills, treatment, and procedures competently and within the regulation and
    licensing laws of the State Nurse Practice Act Institutional policies and other applicable local and
    federal laws.
    m. Performing assessments and reassessments and participating in case conferences.
    2. Work additional hours in the office as assigned by supervisor to a balance of full-time hours (40 total) in
    operational and administrative functions to include:
    a. Mentor, precept and develop field staff in the office and in the home setting.
    b. Onboard new nursing hires for orientation, competency check offs in the office and/or field.
    c. Assures direct care staff functions provide compliant care.
    d. Provides precepting and guidance to agency direct care staff regarding orders for disciplines
    and/or treatments and services, consistent with agency policy.
    e. Develops and implements appropriate treatment programs; develops realistic personal goals
    with patient; re-evaluates, sets new goals, adjusts care accordingly and evaluates for discharge
    potential in consultation with physician or intermediate care provider.
    f. Completes supervisory visits as assigned, comprehensive assessment/reassessment visits per
    Agency policy, federal and state rules, and documenting the visits.
    g. Conducts coordination of care between direct care staff, and external providers.
    h. Serves as liaison between this Agency and physicians, hospitals, and other agencies regarding
    patient care.
    i. Provides positive attitude and example of professional behavior for direct care staff.
    j. Provides orientation and competency verification of direct care staff.
    k. Provides direct, in field, supervision and precepting of newly hired clinicians and assures
    supervision of all staff is completed per regulations and agency policy including documentation
    of same.
    l. Provides direct response and guidance to staff for problem resolution in patient care.
    m. Provides information and reports to Director of Clinical Services/Administrator as instructed.
    n. Participates, as requested, in orientation of new staff, in-service education, continuing education,
    utilization review and QAPI programs.
    o. Participates in/attends staff development activities and in-service meetings to educate the
    personnel.
    p. There may be times operational and administrative duties extend beyond office hours; this can
    include things such as Admissions, Meet and Greets, competency review of nurses, hiring events
    and other operational requirements as they arise.

    ABILITIES, SKILLS AND KNOWLEDGE:
    1. Understanding of the Medicare and Medicaid regulatory guidelines and state licensure.
    2. Expertise in coordinating work activities of multi-disciplinary team.
    3. Excellent oral and written communications skills with proven ability to communicate with technical and
    non-technical users.
    4. Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and
    legal documents.
    5. Ability to read and interpret physicians' orders and notes from other caregivers as well as respond to
    them verbally and in writing.
    6. Ability to effectively present information to clients, employees and community members.
    7. Ability to respond timely and effectively to the most sensitive inquiries or complaints.
    8. Ability to define problems, collect data, establish facts, and draw valid conclusions.
    9. Ability to send/receive emails and use the internet for research and informational purposes.
    10. Ability to use word processing software for communication and documentation purposes.
    11. Ability to use independent judgment to manage and impart confidential information.
    12. Skill to attend to multiple tasks at once which are time sensitive.
    13. Basic knowledge of Windows based operating systems, Electronic Health Records and ability to advance
    computer skills as required
    14. Ability to establish and maintain effective working relationships with fellow employees and the public.

    EXPERIENCE REQUIRED:
    1. Proven supervisory and communication skills.
    2. Proven ability to act as liaison among staff and community.
    3. Case Management/Supervisory experience preferred

    QUALIFICATIONS:
    1. Current state RN licensure without stipulations
    2. Current TB skin tests upon hire and screening annually there-after;
    3. Possession of a valid state Driver's License with reliable transportation and applicable car insurance.
    4. Experience in healthcare of at least two (2) years with a minimum of two (2) years in home health
    preferred.

    PHYSICAL REQUIREMENTS:
    1. Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands
    and arms and talk and hear.
    2. Ability to frequently use hands to finger, handle, or feel.
    3. Ability to occasionally climb.
    4. Ability to occasionally use sense of smell.
    5. Ability to use close vision, distance vision, color vision, peripheral vision, depth perception.
    6. Ability to adjust focus.

    WORK ENVIRONMENT:
    1. The employee is frequently exposed to outside weather conditions
    2. The work environment is highly variable, as care is provided in private homes.
    3. Lighting, acoustics, air quality, temperature and environmental hazards are not predictable.
    4. The employee is occasionally exposed to wet and/or cold conditions, household pets/animals, fumes or
    airborne particles, and toxic or caustic chemicals.
    5. The noise level in the office work environment is usually moderate.

    TRAVEL:
    1. The employee is required to travel frequently to complete agency business.
