Clinical Coordinator Needed! Aspire Home Healthcare is seeking an experienced and dynamic clinical coordinator to assist with the coordination of care through the coordination of various patient, physician, and family activities and communications, conduct home care visits and assist with administrative needs our Yuba City office.
Duties Include:
· Receiving telephone calls and triaging situation from field staff when needed
· Advising skilled field staff of Aspire policies and procedures
· Enter appropriate documentation regarding various patient and/or staff issues
· Serving as a liaison between patient, family, physician, laboratories and insurance companies
· Researching issues regarding patient care and coordinating between the designated parties to resolve the issue
· Serving as a clinical resource for skilled field staff
· Assisting the skilled field staff with obtaining and/or follow-up of physician orders
· Interviewing prospective employees, both field staff and other clinical coordinators
· Assisting with orientation of new clinical coordinators and skilled staff
· Reviewing skilled staff documentation and educating/advising staff of appropriate documentation for various situations
· Reviewing and approving all SOC, ROC and Discharge documentation for appropriateness
· Reviewing and approving all Oasis information for appropriateness
· Taking “On Call” to triage issues, support staff with concerns, staffing needs after hours & monitoring fax mailbox
· Assisting with in-service education
· Conducts performance evaluation of skilled field staff
· Reviewing all interim orders, and 485’s for appropriateness and signing off of each
· Work majority of the time with specific assigned teams but cover for all other clinical coordinators when needed or assigned
· Other projects as requested
